This is done through the Patient Centered Medical Home approach to improve population health, deliver a better patient experience, and provide more affordable care. Our providers see incentives for providing preventive care that saves money and resources. Our providers are engaged in initiatives that increase access and improve healthcare for patients while also improving the overall value.
